Parents of incoming Kindergarten students, please fill out the form below and we will email your child's psms95x.org account username and password.

If you do not know your child's login username or password, please review the information below.

• Before sending a request, confirm that your device is connected to wifi. If you are using a Chromebook but the wifi is not connected, you will not be able to log in (watch this video to assist). 
• Send an email to: [email protected] & or [email protected] with the following information:
  • ​Your child's first and last name
  • OSIS or student ID number
  • Class Number
  • Your name and relationship to the child (you must be the legal guardian to make this request)
