DAILY HEALTH SCREENING
​All employees, visitors, and families seeking to enter DOE buildings must complete a health screening. This screening must be completed on each day of arrival and results will reset at midnight of each day. Upon entering the facility you will be asked to provide the results of your screening either by showing your phone or a printout of the results. CLICK HERE
 COVID-19 TESTING CONSENT FORM
Any student in grade 1 or higher returning to school buildings must submit a consent form for COVID-19 testing in school by their first day back in school buildings. For more testing forms and more information,

REMOTE LEARNING DEVICE REQUESTS:
If you need a device for your child to access the Internet, please enter your information at the registration link below. We will contact you by email to inform you of a pickup date and time. ​CLICK HERE
CREATE / RETRIEVE DOE STUDENT ACCOUNTS
If you have not already done so, please create / retrieve your child's DOE student account using the link below. This account is separate from the psms95x.org account that is used to log into a school Chromebook and access Google Classroom. The DOE account can be used to access TeachHub as well as other citywide resources.  CLICK HERE

APPLICATION FOR KINDERGARTEN STUDENTS 
If your child was born in 2016 and you live in New York City, they can start kindergarten in September 2021. All families, including current pre-K students, students with disabilities, and English Language Learners, should submit an application.

LEARNING BRIDGES
REMOTE LEARNING DAYS CHILD CARE

 The DOE and the Department of Youth and Community Development (DYCD) are collaborating to launch Learning Bridges - a new program that will provide free child care options for children from 3-K through 8th grade from 8am -3pm on days when they are scheduled for remote learning. There are over 200 Learning Bridge sites around the City and each site will be matched with at least one school to recruit students.  We are excited to inform you that your school has been paired with the below Learning Bridges site.

It is with great sadness that we announce the passing of Mrs. Omara Flores, our educator, administrator, and colleague. Mrs. Flores has touched many lives for more than two decades at PS/MS 95. During her years of service she has always demonstrated professionalism, integrity, empathy and devotion to our PS/MS 95 Community. Her tremendous legacy will continue to live on through all of us. Let's keep her family in our prayers. She will be sorely missed.
 
Es con gran tristeza que anunciamos el fallecimiento de la Sra. Omara Flores, nuestro educadora, administradora, y colega. La Sra. Flores ha tocado muchas vidas durante más de dos décadas en PS/MS 95. Durante sus años de servicio, siempre ha demostrado profesionalismo, integridad, empatía y devoción a nuestra comunidad PS/MS 95. Su tremendo legado continuará viviendo a través de todos nosotros. Mantengamos a su familia en nuestras oraciones. Ella será muy extrañada.
