PS/MS 95 is fortunate to have the Montefiore School Based Health Clinic. It provides coordinated primary and preventive healthcare to students. The clinic partners with students’ primary and specialty providers to service the health needs of each individual student who is registered. Since the goal is to create a healthier school and community environment, students are educated as they approach adolescence to empower them to learn to take responsibility for their own care.

A nurse practitioner or a physician along with licensed nurses provides the medical services. At PS/MS 95, a physician provides the services with the assistance of a nurse. Students can be seen for preventive care, physical exams, immunizations, hearing and vision testing or psychosocial and health risk behavior screenings, sore throats, headaches, cold and allergy symptoms, menstrual cramps, first aid, evaluation for injuries/emergencies, and management for chronic illnesses (asthma, diabetes & obesity). Referrals are made to subspecialists as needed.

PS/MS 95’s Advocacy Day is an annual trip to the State Capital in Albany where students explain the importance of the clinic to state legislators and encourage them to allocate funding for the clinic.

The Family Adventure Program

The Family Adventure Program is a program to engage the families and students of P.S./M.S. 95  through monthly trips to various cultural institutions throughout New York City.
 
 Trips are one Saturday a month and are free of charge! If you are interested in signing up, please contact the clinic for more information.
